Tracker
What it does
Tracker is your home health plan. It shows the plan your doctor confirmed, your target ranges, and a place to log readings and mark medicines taken.
When to use it
Use it every day if your doctor has set up a plan — for blood pressure, blood glucose or weight.
Step by step

Sign in, then open Tracker.

Read your plan and the target ranges the doctor set.

Log a reading — blood pressure, pulse, glucose or weight — when you take it.

Each day, mark your prescribed medicines as Taken, Missed or Skipped.

When you want the clinic to have your numbers, press Send readings now.
Also on this screen:
- The target display shows ranges, not scores. It does not grade you.
- The app can remind you in-app to take medicines and log readings.
What Agent D can do
- Agent D works at the clinic. Your readings reach the clinic when you send them.
Safety (what needs your Confirm)
- Send readings now puts your readings on your clinic chart. Until you press, they stay on your phone.
- The targets come from your doctor. If a reading is far outside the range, contact the clinic — do not wait for the app to alarm.
Common questions
Who set the targets? Your doctor, as part of a monitoring plan they confirmed. What if I miss a day? Mark the medicine Missed when you remember, and keep going. Does the clinic see everything right away? Only what you send with Send readings now.
