Facts from a file

What it does

When external documents (such as specialist letters, hospital discharge summaries, or imaging reports) are attached in chat or uploaded to a chart, Agent D reads the text and extracts key clinical facts. It presents extracted measurements, diagnoses, and allergies on a Document facts card with checkboxes. You tick the relevant facts and click File selected to record them.

When to use it

Use Document facts when processing incoming clinical correspondence to populate patient medical histories without manual data entry.

What Agent D can do

Agent D scans document text, identifies clinical measurements (blood pressure, pathology values) and diagnoses, and formats structured facts. It cannot file data without confirmation.

Safety (what needs your Confirm)

No extracted fact is written to the medical record automatically. You must review each extracted fact, tick the desired items, and confirm filing.

Common questions

Can I untick facts I do not want to record?

Yes. Simply uncheck any item that is redundant, historical, or clinically irrelevant.

Does uploading a file save it to the chart immediately?

The file is uploaded for analysis, but extracted data is only recorded to the chart after you click File selected and confirm.

What file formats can Agent D read?

Agent D reads PDF documents, Word files, plain text, and CSV files.
