Records
What it does
Records shows what the clinic has shared with you: your clinical notes, conditions, medicines, allergies, results, letters and plans.
When to use it
Use it to check your medicines, read a result the doctor released, or download a letter.
Step by step

Sign in, then open Records.

Read each part. The clinical notes and conditions are read-only — they are the clinic's record of your care.

To add something of your own, use Add reported medication or Add reported allergy, or upload a document.
Also on this screen:
- The medicines list shows your active prescription medicines with brand name, dose and how often.
- You can log a medicine you take at home that was not prescribed.
- The allergy list shows confirmed allergies and intolerances, and how bad the reaction was.
- A newly noticed allergy or reaction goes to your GP to review — it is not added to the clinic list by itself.
- Results and imaging reports show when the doctor releases them.
- Referral letters, care plans and certificates can be downloaded.
What Agent D can do
- Agent D works at the clinic. What you add here goes to the clinic for a doctor to review.
Safety (what needs your Confirm)
- The clinic record is read-only for you. If something is wrong, tell the clinic.
- Anything you add is marked as reported by you, and a doctor reviews it before it becomes part of your record.
Common questions
Why can't I see my latest result? Results appear when the doctor releases them. Ring the clinic if you are waiting. Can I fix a wrong medicine on the list? Not directly. Add it as a reported medicine or tell the clinic, and a doctor will sort it. What can I upload? A PDF or a photo of outside records, like immunisation records from another clinic.
